About the Role

As a Home Health LPN with CenterWell Home Health, you will play a vital role in providing high-quality care to patients in the comfort of their own homes. Our team is dedicated to delivering personalized, comprehensive care that addresses the physical, emotional, and social needs of our patients.

Key Responsibilities
  • Provide professional nursing services under the direction of a RN, ensuring compliance with state regulations and CenterWell Home Health policies.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Collaborate with the healthcare team to ensure seamless care coordination and effective communication.
  • Confer with the RN Supervisor to discuss patient progress and make necessary adjustments to the care plan.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
