Job Summary

CenterWell is seeking a highly skilled Patient Services Coordinator LPN Home Health to join our team. As a key member of our home health care team, you will be responsible for scheduling visits and communicating with field staff, patients, physicians, and other stakeholders to ensure seamless care coordination and continuity of care.

Key Responsibilities
  • Schedule visits and manage patient schedules to ensure timely and efficient care delivery
  • Communicate with field staff, patients, and physicians to resolve scheduling conflicts and ensure continuity of care
  • Initiate infection control forms as needed and maintain accurate records
  • Assist with day-to-day office and staff management, including scheduling and rescheduling visits
  • Run scheduling reports and maintain accurate records of patient information
  • Collaborate with the Branch Director to ensure smooth operation of the home health care team
Requirements
  • Current LPN licensure in the state of employment
  • At least 1 year of home health experience
  • Prior packet review/QI experience preferred
  • Coding certification is preferred
  • Excellent communication and organizational skills
  • Ability to manage and prioritize multiple assignments
