About the Role:As a Home Health Registered Nurse, you will be responsible for providing skilled nursing visits to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Key Responsibilities:- Conduct comprehensive assessments of patients to identify their healthcare needs and develop personalized care plans.
- Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
- Collaborate with other healthcare professionals, including physicians, therapists, and home health aides, to ensure seamless care coordination.
- Communicate effectively with patients, families, and healthcare teams to ensure that care plans are met and that patients receive the highest level of care.
- Document patient progress and outcomes in accordance with regulatory requirements and company policies.
- Diploma, Associate, or Bachelor's degree in Nursing.
- Minimum of one year of nursing experience, preferably in a home health or hospice setting.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver's license and reliable transportation.
