We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities- Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitation, recovery, and preventative care.
- Collaborate with the healthcare team to ensure seamless care coordination and effective communication with patients, families, and healthcare providers.
- Conduct thorough assessments, identify patient needs, and develop strategies to address those needs in a holistic and patient-centered approach.
- Document patient progress, communicate with the healthcare team, and maintain accurate records in accordance with CenterWell Home Health standards.
- Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
