Job Description

Lutheran Social Services of the Southwest is a nonprofit organization dedicated to assisting vulnerable community members. We are seeking a highly skilled Care Coordinator for our Aging and Disability Services program.

About Us

We deliver services that stabilize during crises, build self-reliant foundations, and preserve dignity and respect. Our team supports over 5,000 Arizonans across Metro Phoenix, Tucson, and southern Arizona through programs in Refugee Services, Aging & Disability Services, Family Services, and Emergency Services.

Job Responsibilities
  • Assist with supervisory home visits, client intakes, and employee observation visits.
  • Record and accurately communicate information between clients, Direct Care Workers, Case Managers, and the Home Care Supervisor.
  • Manage accurate and timely data entry into the system for billing and payroll.
  • Ensure employee credentials are current and manage client and employee schedules.
  • Provide backup coverage for Direct Care Workers when needed.
Requirements

To be successful in this role, you should have at least six months of experience working with individuals with disabilities or elderly clients. You must also possess certification as a Direct Care Worker, including knowledge of caregiving principles and physical disabilities.

Benefits

This is a full-time position with an estimated annual salary of $45,000 - $55,000, depending on experience. Benefits include competitive pay, comprehensive health insurance, and opportunities for professional growth and development.
