Hospice Nurse/Case Manager

DEPENDABLE HOSPICE AND PALLIATIVE CARE is seeking a dedicated and compassionate Hospice Nurse/Case Manager to join our team.
Key Responsibilities:
- Care Coordination: Develop and implement individualized care plans for patients, ensuring seamless coordination with healthcare teams and family members.
- Ongoing Evaluation: Continuously assess the effectiveness of hospice services, making adjustments as needed to ensure optimal patient outcomes.
- Patient Assessments: Conduct thorough evaluations to determine the best course of care for patients, including informational visits as required.
- Nursing Process: Utilize the nursing process to deliver high-quality, patient-centered care, adhering to state scope of practice and agency policies.
- Consultation: Collaborate with attending physicians and the Hospice Medical Director to ensure accurate updates to patient care plans.
- Scheduling: Submit weekly patient visit schedules to ensure efficient care delivery.
- IDG Participation: Engage in Interdisciplinary Group meetings and case conferences to foster a comprehensive understanding of patient needs.
- On-Call Duties: Provide on-call support, including holidays, in accordance with agency policies.
- Patient Education: Educate patients and family members on the dying process, care plans, and healthcare decision-making guidance.
- Dietary Counseling: Offer dietary advice, taking into account patients' health issues and end-of-life dietary needs.
- Death Visits: Conduct death visits as per agency policy, providing support and care during this sensitive time.
- Health Teaching: Assess patients, family members, and home environments to identify necessary health teaching.
- Plan of Care Development: Develop and implement initial care plans, collaborating with IDG members to maintain comprehensive care plans.
- Supervision: Supervise and evaluate the care provided by Hospice Aides, ensuring adherence to agency policies and standards.
- LPN Oversight: Guide and supervise LPNs, ensuring adherence to state scope of practice and agency policies.
- Care Plan Preparation: Prepare care plans for Hospice Aides, ensuring accurate and timely delivery of care.
Qualifications:
- Education: Must be a graduate of an accredited School of Nursing.
- Licensure: Must hold a valid Arizona Registered Nurse license.
- Experience: Minimum of one year of RN experience; hospice experience preferred.
- Assessment Skills: Excellent assessment and problem-solving skills.
- Organizational Skills: Strong organizational abilities and time management knowledge.
- Driving Requirements: Must be a licensed driver with an insured automobile in good working order.
- Communication: Strong verbal and written communication skills.
- Clinical Competence: No exceptions on RN license; ability to administer medications, perform medication reconciliation, and carry out nursing procedures or treatments as needed within state scope of practice.
