The Quality Coordinator/RN is a pivotal role within the Northern Nevada Health System, dedicated to enhancing patient care through effective coordination and management of healthcare services. This full-time position offers a comprehensive benefits package and operates on a convenient day shift.

Role Responsibilities

The Quality Coordinator/RN acts as a vital link between patients and the multidisciplinary healthcare team, focusing on the development and implementation of patient-centered care plans for high-risk individuals. Key responsibilities include:

1. Engaging with high-risk patients within two business days post-discharge to ensure a smooth transition.
2. Evaluating community resources available to patients and coordinating necessary services.
3. Formulating individualized care plans aimed at preventing hospital readmissions and emergency department visits.
4. Assisting clinics in scheduling follow-up visits within 7 to 14 days post-discharge.
5. Conducting follow-up assessments with patients for 30 days after discharge as clinically required.

Qualifications

To be considered for this role, candidates must possess:
- Graduation from an accredited nursing program and a valid state license as a Registered Nurse.
- Preferred training in Motivational Interviewing.
- A Bachelor’s Degree; a Master’s Degree is preferred.
- A minimum of five years of experience in case management, home care, or hospice.
- Knowledge of Motivational Interviewing, Shared Decision Making, and Patient-Centered Medical Home principles.
- Certification as a Certified Case Manager (CCM) is advantageous.
