Job Summary

The Quality Coordinator/RN is a registered nurse who functions in a leadership role in Population Health. This position is responsible for coordinating patient care and developing plans to improve health outcomes.

Key Responsibilities

  • Assess patients' needs and coordinate resources to promote optimal health benefits and quality outcomes.
  • Work with the local Quality Team to analyze and communicate population health data to meet the Triple Aim of Population Health.
  • Contacts high-risk patients within 2 business days of discharge to ensure a safe transition.
  • Assesses for community resources at home and arranges for services as needed.
  • Creates patient-centered care plans to avoid hospitalizations and emergency department visits.
  • Assists the clinic in scheduling the Transition of Care (TOC) visit within 7-14 days.

Requirements

  • Graduation from an accredited nursing education program.
  • Training in Motivational Interviewing preferred.
  • Bachelor's Degree.
  • Minimum of 5 years in case management, home care, or hospice required.
  • Critical thinking, problem-solving, ability to work both independently and in teams, and ability to coach and mentor clinical and non-clinical staff.
