Position Overview

The Quality Coordinator/RN role is a full-time position that provides comprehensive benefits and operates on a convenient day shift. This registered nurse assumes a pivotal leadership role within Population Health, acting as a crucial link between patients and the multidisciplinary healthcare team.

Key Responsibilities:
  • Engage with high-risk patients within two business days post-discharge to ensure a smooth transition.
  • Evaluate community resources available to patients at home and facilitate necessary services.
  • Develop individualized care plans aimed at preventing hospital readmissions and emergency department visits.
  • Assist clinics in scheduling follow-up visits within 7 to 14 days.
  • Conduct follow-up assessments with patients for up to 30 days after discharge as clinically indicated.

Qualifications:
  • Graduation from an accredited nursing program.
  • Current licensure as a Registered Nurse in the state.
  • Preferred training in Motivational Interviewing.
  • Bachelor's Degree required; Master's Degree preferred.
  • A minimum of 5 years of experience in Case Management, Home Care, or Hospice.
  • Familiarity with Motivational Interviewing, Shared Decision Making, and Patient-Centered Medical Home principles.
  • Certification as a Certified Case Manager (CCM) is a plus.

  • Capacity to work independently and collaboratively within teams.
  • Ability to coach and mentor both clinical and non-clinical staff.
