Position Overview

The Quality Coordinator/RN role is a full-time position that provides comprehensive benefits and operates on a convenient day shift. This position is designed for a registered nurse who will take on a leadership role within the realm of Population Health.

Independence Physician Management (IPM), a subsidiary of UHS, was established in 2012 to serve as the physician services unit of UHS. IPM focuses on developing and managing multi-specialty physician networks and urgent care clinics that align with UHS acute care facilities. It also provides select services for the Behavioral Health division of UHS. With ongoing growth, IPM now operates in 11 markets across six states and the District of Columbia.

Key Responsibilities:
  • Contact high-risk patients within two business days post-discharge to ensure a smooth transition.
  • Evaluate community resources available at home and coordinate necessary services.
  • Develop a patient-centered care plan aimed at preventing hospitalizations and emergency department visits.
  • Assist the clinic in scheduling follow-up visits within 7 to 14 days.
  • Conduct follow-ups with patients for 30 days post-discharge as clinically warranted.

Qualifications:
  • Graduation from an accredited nursing education program.
  • Current licensure as a Registered Nurse in the state.
  • Preferred training in Motivational Interviewing.
  • Bachelor's Degree required; Master's Degree preferred.
  • A minimum of five years of experience in case management, home care, or hospice is required.
  • Knowledge of Motivational Interviewing, Shared Decision Making, and Patient-Centered Medical Home principles.
  • Certification as a Certified Case Manager (CCM) is preferred.
  • Strong critical thinking and problem-solving skills, with the ability to work independently and collaboratively.
  • Ability to coach and mentor both clinical and non-clinical staff.
