Job Summary

Dartmouth Health is seeking a skilled Community Health Resource Specialist to join our team. As a key member of our healthcare team, you will play a vital role in supporting patients, families, and community members in accessing and utilizing resources related to Social Determinants of Health (SDoH).

Key Responsibilities

  • Assist patients and families in identifying and accessing resources, information, and education related to SDoH.
  • Provide support and guidance to healthcare providers in clarifying the need for services and determining the availability of required resources.
  • Visit patients at home or community locations as needed to provide support and connect them with available resources.
  • Update and maintain the Community Resource Directory and utilize the RedCap database for tracking interactions and documentation in EPIC EMR.
  • Implement evidence-based curriculum as needed to support patient and family needs.

Requirements

  • Bachelor's degree in Human Services or related field preferred.
  • Excellent interpersonal, communication, and organizational skills required.
