Resource Coordinator

Job Summary
We are seeking a skilled Resource Specialist to join our team at Dartmouth Health. In this role, you will provide critical support to patients and families by identifying and facilitating access to essential services and resources.
Key Responsibilities
- Act as an advocate for patients and families to obtain necessary services and resources.
- Update and maintain information resource directories and related literature for distribution to staff and families.
- Document services provided in patient medical records and applicable databases.
- Negotiate access to skilled, acute, or long-term care facilities and identify transportation resources.
- Assess patient eligibility for medication, co-pay, and additional resource assistance.
- Facilitate applications to public and private government and community agencies, primarily Medicaid.
- Provide information and assistance to secure financial and health assistance, organization financial assistance, and other governmental programs.
Requirements
- Bachelor's degree in Social Work or Human Service field or equivalent education and experience.
- Excellent interpersonal, communication, and organizational skills.
