Job Summary

Dartmouth-Hitchcock Health is seeking a skilled Healthcare Resource Coordinator to join our team. In this role, you will play a critical part in assisting patients, families, and healthcare providers in accessing and utilizing resources related to Social Determinants of Health (SDoH).

Key Responsibilities

  • Reach out to patients identified by SDoH screenings and referrals to provide assistance in identifying and referring to resources.
  • Meet with patients to assess their needs and determine the availability of required services.
  • Provide patient information and support concerning available resources and assist patients in obtaining appropriate resources.
  • Update the Community Resource Directory and utilize the RedCap database for tracking interactions and documentation in EPIC EMR.
  • Implement evidence-based curriculum as needed and identify and contribute to the development of systems and resources needed to meet the support needs of patients, families, and staff.
  • Assist in marketing Community Health work and Social Determinants of Health impact to providers and the system.

Requirements

  • Bachelor's degree in Human Services or a related field preferred. High School diploma/GED with equivalent experience considered.
  • Excellent interpersonal, communication, and organizational skills required.
