Senior Care Coordinator

Home Instead Senior Care is seeking a highly motivated and compassionate Client Care Coordinator to join our team. As a Client Care Coordinator, you will play a vital role in providing exceptional care and support to our clients and their families.
Key Responsibilities:- Conduct care consultations with potential clients and family members to determine their needs and provide solutions.
- Develop and implement personalized service plans to meet the unique needs of each client.
- Coordinate with other team members to ensure seamless delivery of care services.
- Conduct quality assurance visits to ensure high-quality care and client satisfaction.
- Communicate effectively with clients, family members, and care professionals to ensure open and transparent communication.
- College degree preferred.
- One year of experience in home care, healthcare, or a senior-related industry.
- CNA or nurse aide experience a plus.
- Valid driver's license.
