Key Responsibilities- Implement and monitor patient care plans to ensure effective and efficient care.
- Monitor, record, and communicate patient condition as appropriate to ensure timely and accurate care.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care, ensuring seamless communication and collaboration among healthcare teams.
- Carry out physician and nursing orders, ensuring accurate and timely execution of care plans.
- Assess and coordinate patient's discharge planning needs with patient/family and members of the healthcare team, ensuring a smooth transition to post-acute care.
- Assess learning needs of patient and family and provide education based on age, culture, and willingness to learn, promoting patient empowerment and self-care.
- Perform job duties in accordance with the Nurse Practice Act, adhering to professional standards and guidelines.
- Associate's degree or higher or Diploma in Nursing required upon hire.
- Bachelor's degree or higher in Nursing required within 5 years of employment in this position.
- No experience required, but RN - Licensed as a Registered Nurse and eligible to practice nursing in the State of Kentucky required upon hire.
- BLS - Certified as a Basic Life Support Healthcare Provider required within 45 days of employment in this position.
- Requires critical thinking skills and decisive judgment to make informed decisions in a fast-paced environment.
- Works under minimal supervision, demonstrating autonomy and self-motivation.
- Must be able to work in a stressful environment and take appropriate action to ensure patient safety and well-being.
