Position Summary for Nurse Practitioner (NP):

The Palliative Care Nurse Practitioner (NP) collaborates with the American Veterans Hospice team to provide comprehensive, high-quality hospice and palliative care services under the supervision of the Medical Director. The NP will conduct patient visits for both new and established palliative care patients and coordinate their care plans with designated hospice staff, particularly the palliative care team. The NP will be an essential member of the hospice provider team and will fulfill the requirements of the 'face-to-face encounter' with Medicare and Medicaid beneficiaries enrolled in our hospice services as needed. Patient visits may occur in homes, facilities, hospitals, and the Hospice Inpatient Unit. Additionally, the NP will engage in independent decision-making regarding patients' healthcare needs and provide hospice or palliative care services as required. Consultation, collaboration, or referral will be available from American Veterans Hospice medical directors.

Essential Duties and Responsibilities:

  • Provide medical support and coordination for palliative care patients and their families, including expertise in pain and symptom management and evaluating terminal prognosis.
  • Ensure that care adheres to established hospice and palliative care guidelines and standards.
  • Collaborate with attending physicians regarding patient care and treatment within the Palliative Care program and facilitate potential referrals to hospice.
  • Conduct patient visits primarily in private residences, long-term care facilities, assisted living facilities, hospitals, or other locations.
  • Oversee pharmacy formulary and medication expenditures.
  • Document face-to-face encounters for the Medical Director, including clinical findings supporting a life expectancy of six months or less.

Knowledge, Skills, and Abilities Required:

  • Proficient in electronic medical records and patient history acquisition.
  • Understanding of health education theory and practice.
  • Familiarity with clinical operations and procedures.
  • Ability to uphold quality, safety, and infection control standards.
  • Knowledge of current principles and methods for medical evaluation, diagnosis, and treatment in hospice and palliative care.
  • Strong interpersonal and communication skills, capable of working effectively with diverse constituencies.
  • Awareness of related accreditation and certification requirements.
  • Understanding of Medicare 'Face to Face Encounter' criteria.
  • Knowledge of the interdisciplinary team concept.
  • Ability to collaborate with referring, primary, and consulting physicians and other interdisciplinary team members to provide appropriate palliative care.
  • Experience working with healthcare providers in long-term care settings.
  • Demonstrated leadership skills.
  • Excellent organizational abilities.
  • Competence in handling detailed and confidential information.
  • Computer proficiency is required.

Other Responsibilities:

  • Availability for weekend face-to-face encounters as needed.
  • May be required to visit active patients or new admissions after hours or on holidays for symptom or wound management as directed by the Director of Hospice or Medical Director.

Minimum Qualifications for Nurse Practitioner (NP):

  • MSN from an accredited nursing institution; current licensure as a Registered Nurse and Nurse Practitioner in good standing.
  • 2-4 years of experience in a medical specialty related to the care of patients with serious, advanced, or terminal illnesses.
  • Ability to travel to various territories.
  • Possession of a DEA license.
  • A minimum of 2 years of clinical experience in Oncology, Pain & Symptom Management, Palliative Care, and Hospice within a hospital or physician practice setting.
