Work Schedule:
Full-time
Monday – Friday, 8:30 am – 5:00 pm

Key Responsibilities:
- Provide comprehensive case management services to clients facing challenges related to homelessness, mental health, and substance use.
- Develop and implement individualized service plans tailored to client needs.
- Collaborate with community resources and agencies to ensure clients receive necessary support.
- Maintain accurate and timely documentation of client interactions and progress.
- Foster a supportive and empathetic environment for clients.

Qualifications:
- A minimum of one year of relevant experience in social services, particularly with populations experiencing homelessness or mental health issues.
- Valid driver's license is required.
- Strong proficiency in computer applications, including Microsoft Word, Excel, and various web-based tools.
- Must demonstrate flexibility, reliability, and a positive demeanor.
- Excellent organizational skills with the ability to manage multiple tasks effectively.
- Capable of working independently as well as collaboratively within a team.

Compensation:
- Competitive pay rate of $23/hour.
