Clinical Triage Nurse

SUMMARY: The mission of NACC is to enhance the health and wellness of Native American families by addressing their physical, mental, and spiritual needs. We provide comprehensive healthcare services, including Medical, Behavioral Health, Dental, and Substance Abuse support. The ideal candidate will have a background in nursing triage and experience in community healthcare settings. This role is accountable to the Nursing Supervisor and requires excellent communication and organizational abilities, along with effective problem-solving skills.
KEY RESPONSIBILITIES:- Manage incoming patient inquiries and perform triage for both phone and in-person visits, including communicating critical lab results and provider directives.
- Evaluate patient concerns across all age demographics via health data collection.
- Develop, prioritize, and implement treatment plans based on established protocols.
- Process prescription refills in accordance with company guidelines.
- Conduct follow-up calls post-hospitalization.
- Assist in the creation and revision of written protocols.
- Collaborate with NACC leadership on nursing-related projects.
- Participate in regular meetings with the Medical Director, care coordinators, and medical teams for planning and reporting purposes.
- Perform additional duties as assigned by Medical Leadership.
This position does not have supervisory responsibilities.
QUALIFICATIONS:Education and Experience: A minimum of two years of direct patient care experience in a clinical environment, preferably with a focus on nursing triage. Experience working with Native American or other underserved communities is advantageous.
Licenses and Certifications: Must possess a valid Registered Nurse (RN) license with no restrictions in Minnesota, BLS CPR certification, and proof of current COVID-19 vaccination.
Technical Skills: Proficiency in Internet Software, Spreadsheet Software (Excel), Word Processing Software (Word), Electronic Mail Software (Outlook), Presentation Software (PowerPoint), and familiarity with EMR systems is preferred.
Interpersonal Skills: Ability to work independently and collaboratively within a team, along with exceptional communication, customer service, and case management skills.
