Patient Care Triage Nurse

POSITION SUMMARY:
The Native American Community Clinic is dedicated to enhancing the health and well-being of Native American families. We provide comprehensive healthcare services, including Medical, Behavioral Health, Dental, and Substance Abuse care. The successful candidate will have a solid background in clinical community care and nursing triage.
This role is under the supervision of the Nursing Supervisor. Candidates should demonstrate exceptional communication and organizational abilities, along with effective problem-solving skills in a workplace setting.
KEY RESPONSIBILITIES:
Manage incoming patient communications and triage (both phone and in-person), including the relay of abnormal lab results and other directives from healthcare providers.
Plan, prioritize, implement, and recommend treatment options in accordance with established protocols.
Process prescription refills following company guidelines.
Conduct follow-up calls for hospital discharges.
Assist in the creation and revision of written protocols.
Collaborate with the leadership team to execute projects relevant to the nursing department.
Participate in regular meetings with the Medical Director, care coordinators, and medical teams for planning and reporting purposes.
Perform other related tasks as assigned by Medical Leadership.
SUPERVISORY ROLE:
This position does not include supervisory responsibilities.
QUALIFICATIONS:
Education and Experience:
A minimum of 2 years of direct patient care experience in a clinical environment, ideally with a focus on nursing triage; prior experience with Native American or other underserved communities is preferred.
Required Certifications and Licenses:
Registered Nurse (RN) with an active, unrestricted license to practice in Minnesota; BLS CPR certification; proof of current COVID-19 vaccination.
Technical Skills:
Proficiency in Internet Software, Spreadsheet Software (Excel), Word Processing Software (Word), Electronic Mail Software (Outlook), Presentation Software (PowerPoint); experience with EMR systems is preferred.
Additional Skills:
Ability to work independently and collaboratively; strong communication, customer service, and case management skills.
