SUMMARY: The mission of NACC is to enhance the health and wellness of Native American families, focusing on the holistic well-being of mind, body, and spirit. We provide comprehensive healthcare services encompassing Medical, Behavioral Health, Dental, and Substance Abuse. The ideal candidate will have clinical experience in community healthcare and a background in nursing triage. This role reports to the Nursing Supervisor and requires strong communication and organizational abilities, along with effective problem-solving skills in the workplace.
KEY RESPONSIBILITIES:- Manage patient inquiries via phone and in-person triage, including communicating abnormal lab results and provider directives.
- Evaluate patient concerns across all age demographics through the collection of relevant health information.
- Plan, prioritize, implement, and recommend treatment options based on established protocols.
- Process prescription refills in accordance with company guidelines.
- Conduct follow-up calls with patients post-hospitalization.
- Assist in the development and revision of written protocols.
- Collaborate with NACC's leadership team on projects related to nursing operations.
- Participate in regular meetings with the Medical Director, care coordinators, and medical teams for planning and reporting purposes.
- Perform other related duties as assigned by Medical Leadership.
This position does not include supervisory responsibilities.
QUALIFICATIONS:Education and Experience: A minimum of 2 years of direct patient care experience in a clinical environment, preferably in nursing triage; experience working with Native American or other underserved communities is advantageous.
Licenses and Certifications: Registered Nurse (RN) with an active, unrestricted license to practice in Minnesota; BLS CPR certification; proof of current COVID-19 vaccination.
Technical Skills: Proficiency in Internet Software, Spreadsheet Software (Excel), Word Processing Software (Word), Electronic Mail Software (Outlook), Presentation Software (PowerPoint); experience with EMR systems is preferred.
Interpersonal Skills: Ability to work independently and collaboratively within a team; exceptional communication, customer service, and case management skills are essential.
