Patient Triage Registered Nurse

SUMMARY: The mission of NACC is to enhance the health and wellness of Native American families, addressing the holistic needs of mind, body, and spirit. We provide comprehensive healthcare services, including Medical, Behavioral Health, Dental, and Substance Abuse. The ideal candidate will possess clinical experience in community healthcare and a background in nursing triage. This role reports directly to the Nursing Supervisor. Strong communication and organizational skills are essential, along with the capability to effectively resolve workplace challenges.
KEY RESPONSIBILITIES:- Manage incoming patient communications and triage (both phone and in-person), including the communication of abnormal lab results and provider-directed orders.
- Evaluate patient concerns and issues across all age demographics via health data collection.
- Develop, prioritize, implement, and recommend treatment plans in accordance with established protocols.
- Process prescription refills following approved company guidelines.
- Conduct follow-up calls with patients post-hospitalization.
- Assist in the creation and revision of written protocols.
- Collaborate with NACC's leadership team on projects relevant to the nursing department.
- Participate in regular meetings with the Medical Director, care coordinators, and medical teams for planning and reporting purposes.
- Perform other related duties as assigned by Medical Leadership.
This position does not have supervisory responsibilities.
QUALIFICATIONS:Education and Experience: A minimum of 2 years of experience in direct patient care within a clinical environment, preferably with a focus on nursing triage. Experience working with Native American or other underserved communities is preferred.
Licenses and Certifications: Registered Nurse (RN) with an active, unrestricted license to practice in Minnesota; BLS CPR certification; proof of current COVID-19 vaccination.
Technical Skills: Proficiency in Internet Software, Spreadsheet Software (Excel), Word Processing Software (Word), Electronic Mail Software (Outlook), Presentation Software (PowerPoint); experience with EMR systems is preferred.
Interpersonal Skills: Ability to work independently and collaboratively within a team; exceptional communication, customer service, and case management skills are required.
