Clinical Triage Nurse

SUMMARY:
The Native American Community Clinic is dedicated to enhancing the health and well-being of Native American families. We provide comprehensive healthcare services, including Medical, Behavioral Health, Dental, and Substance Abuse support. The ideal candidate will have a background in nursing triage and experience in community healthcare settings.
This role reports directly to the Nursing Supervisor. Candidates should demonstrate exceptional communication and organizational abilities, along with a capacity to effectively address workplace challenges.
KEY RESPONSIBILITIES:
Manage incoming patient communications and triage requests (both phone and in-person), including the dissemination of abnormal lab results and other directives from healthcare providers.
Plan, prioritize, implement, and recommend treatment strategies in accordance with established protocols.
Process prescription refills following approved organizational guidelines.
Conduct follow-up calls post-hospitalization.
Assist in the creation and revision of written protocols.
Collaborate with NACC's leadership team on projects relevant to the nursing department.
Participate in regular meetings with the Medical Director, care coordinators, and medical teams for planning and reporting purposes.
Perform other related tasks as assigned by Medical Leadership.
SUPERVISORY ROLE:
This position does not entail supervisory responsibilities.
QUALIFICATIONS:
Education and Experience:
A minimum of 2 years of direct patient care experience in a clinical environment, preferably in nursing triage; experience working with Native American or other underserved communities is advantageous.
Licenses and Certifications:
Registered Nurse (RN) with an active, unrestricted license to practice in Minnesota; BLS CPR certification; proof of current COVID-19 vaccination.
Technical Skills:
Proficiency in Internet Software, Spreadsheet Software (Excel), Word Processing Software (Word), Electronic Mail Software (Outlook), Presentation Software (PowerPoint); experience with EMR systems is preferred.
Additional Skills:
Ability to work independently and collaboratively; strong communication, customer service, and case management skills.
