Job Summary

The Hospice Aide provides a variety of services to hospice patients and their families, primarily in conjunction with or under the supervision of a Registered Nurse or Physician.

Key Responsibilities

  • Deliver personal care and home maintenance services to patients, as assigned by a healthcare professional.
  • Conduct regular visits to patients, as needed, to assess their needs and provide support.
  • Communicate effectively with interdisciplinary teams to ensure seamless care coordination.
  • Notify patients of scheduled visits and maintain accurate sign-in logs.
  • Escalate concerns to the Clinical Care Manager when patients refuse service or require additional support.
  • Document services provided and maintain accurate records.
  • Participate in team meetings to discuss patient care and share best practices.
  • Assist in the orientation of new Hospice Aides, under the supervision of the Clinical Care Manager.
  • Provide excellent customer service and maintain a positive attitude in a fast-paced environment.

Requirements

  • Active Certified Nursing Assistant certification in the state of Florida.
  • CPR certification.
  • 1-year experience in a healthcare setting.
  • Proficiency in computer office/clinical software.
  • Excellent communication and interpersonal skills.

Physical Demands

  • Ability to lift and/or move up to 50 pounds and push/pull up to 250+ pounds.
  • Ability to stand for extended periods and walk, climb stairs, or ladders as needed.
