Job Summary
The Hospice Aide provides a variety of services to hospice patients and their families, primarily in conjunction with or under the supervision of a Registered Nurse or Physician.
Key Responsibilities
- Deliver personal care and home maintenance services to patients, as assigned by a healthcare professional.
- Conduct regular visits to patients, as needed, to assess their needs and provide support.
- Communicate effectively with interdisciplinary teams to ensure seamless care coordination.
- Notify patients of scheduled visits and maintain accurate sign-in logs.
- Escalate concerns to the Clinical Care Manager when patients refuse service or require additional support.
- Document services provided and maintain accurate records.
- Participate in team meetings to discuss patient care and share best practices.
- Assist in the orientation of new Hospice Aides, under the supervision of the Clinical Care Manager.
- Provide excellent customer service and maintain a positive attitude in a fast-paced environment.
Requirements
- Active Certified Nursing Assistant certification in the state of Florida.
- CPR certification.
- 1-year experience in a healthcare setting.
- Proficiency in computer office/clinical software.
- Excellent communication and interpersonal skills.
Physical Demands
- Ability to lift and/or move up to 50 pounds and push/pull up to 250+ pounds.
- Ability to stand for extended periods and walk, climb stairs, or ladders as needed.
