Overview:

Our team of healthcare professionals works together to ensure that every patient receives personalized attention and care.

Responsibilities:

  • Provide clinical support to HarmonyCares Medical Group patients through phone encounters, answering inbound inquiries and communicating with providers as needed.
  • Maintain a paneled caseload of lower acuity patients and assist with care coordination efforts, including proactive monitoring and outreach.
  • Initiate the SBAR process and utilize triage protocols to assess clinical situations and provide recommendations to patients and providers.
  • Document all phone calls and patient interactions in the EHR, utilizing standard clinical documentation guidelines.
  • Review, transcribe, and communicate providers' verbal orders, as requested.
  • Assist with patients' medication refill requests, checking for duplicate requests and preparing necessary information for provider review.

Requirements:

  • Active LPN/LVN license in the state where patients are located, or equivalent experience.
  • 2 years of experience as an LPN or LVN, with familiarity with Medicare and Medicaid insurances and programs.
  • Proficiency in using electronic health records (EHR) systems, call center technology, and Microsoft Office Suite.
  • Ability to communicate effectively with patients, family members, and facility staff.
