About the Role

We are seeking a skilled and compassionate Community Health Care Assistant to join our team at Tufts Medicine. As a key member of our care team, you will provide personalized support and care to patients in their homes, ensuring they receive the highest level of care and comfort.

Key Responsibilities
  • Provide personal care and assistance to patients, including bathing, dressing, grooming, and toileting.
  • Assist with daily living activities, such as meal preparation, medication management, and mobility support.
  • Monitor patients' physical and emotional well-being, reporting any changes or concerns to the supervising clinician.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Provide education and support to patients and their families on health-related topics and self-care techniques.
Requirements
  • High school diploma or equivalent.
  • Certified Home Health Aide or Licensed Nursing Aide.
  • Valid driver's license.
