Care Manager LCSW FT/Days

As a Care Manager LCSW, you will play a vital role in providing comprehensive care to patients with psychiatric mental health needs. This is a full-time day shift position, requiring a Master's Degree in Social Work and a California Clinical Social Worker License. You will work collaboratively with patients, caregivers, nurses, social workers, physicians, payers, and support staff to facilitate communication and care coordination.
Responsibilities- Provide care management services to patients with psychiatric mental health needs
- Collaborate with interdisciplinary teams to develop and implement care plans
- Facilitate communication and care coordination among patients, caregivers, and healthcare providers
- Conduct assessments and evaluations to identify patient needs and develop care plans
- Provide education and support to patients and caregivers on mental health topics
- Master's Degree in Social Work
- California Clinical Social Worker License
- 2 years of experience in a healthcare setting, preferably in an inpatient setting
- Computer experience
- Discharge planning experience
