Care Manager RN FT/Days

The Care Manager RN FT/Days is a critical member of our healthcare team, responsible for providing exceptional patient care and coordination of services. As a registered nurse, you will work closely with patients, families, and healthcare professionals to ensure seamless care transitions and optimal outcomes.
Key Responsibilities- Assess patient needs and develop individualized care plans
- Coordinate with healthcare teams to ensure timely and effective care delivery
- Provide education and support to patients and families
- Collaborate with social workers and other healthcare professionals to address complex patient needs
- Document patient progress and outcomes
- Bachelor's Degree in Nursing
- California Registered Nurse License
- 2 years of acute hospital experience, including 1 year of supervisory experience
- Strong knowledge base or experience in specialty area assigned
