Key Responsibilities:- Facilitate communication and care coordination along a continuum, ensuring seamless transitions and effective care management.
- Recognize and address the social determinants of health, including barriers to coping, abuse, mental health, substance use, and lack of resources.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Provide education and support to patients and their families, empowering them to make informed decisions about their care.
- Master's Degree in Social Work.
- Current California Clinical Social Worker License.
- Designated as Behavioral Health Clinician to perform 5150 evaluations within 18 months of hire.
- CPI certification (nonviolent crisis intervention training) within 1 year of hire.
- 2 years of experience in a healthcare setting with psychiatric mental health patient population.
