As a Care Manager LCSW, you'll play a vital role in our Department of Psychiatry, working collaboratively with patients, caregivers, nurses, social workers, physicians, payers, and support staff to provide comprehensive care coordination.

Key Responsibilities:
  • Facilitate communication and care coordination along a continuum, ensuring seamless transitions and effective care management.
  • Recognize and address the social determinants of health, including barriers to coping, abuse, mental health, substance use, and lack of resources.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Provide education and support to patients and their families, empowering them to make informed decisions about their care.
Requirements:
  • Master's Degree in Social Work.
  • Current California Clinical Social Worker License.
  • Designated as Behavioral Health Clinician to perform 5150 evaluations within 18 months of hire.
  • CPI certification (nonviolent crisis intervention training) within 1 year of hire.
  • 2 years of experience in a healthcare setting with psychiatric mental health patient population.
