Care Manager RN FT/Day

About the Role
The Care Manager RN FT/Day is a vital member of our healthcare team, responsible for providing exceptional patient care and coordination. As a skilled RN, you will work closely with patients, families, and healthcare professionals to assess, plan, and implement individualized care plans.
Key Responsibilities
- Assess patients' physical, emotional, and social needs
- Develop and implement comprehensive care plans
- Collaborate with healthcare teams to ensure seamless care coordination
- Provide education and support to patients and families
- Monitor and evaluate patient outcomes
Requirements
- Bachelor's Degree in Nursing
- Current California RN License
- 2 years of acute hospital experience, including 1 year of supervisory experience
- Strong knowledge base or experience in specialty area assigned
Preferred Qualifications
- Master's Degree in Healthcare-related field
