Care Manager RN FT/Days

We are seeking a skilled Care Manager RN to join our team at Providence Little Company Of Mary Torrance Hospital. As a key member of our care management team, you will play a vital role in ensuring the highest quality patient care and outcomes.
Key Responsibilities- Assess patient needs and develop individualized care plans to meet their comprehensive health needs.
- Collaborate with healthcare professionals to coordinate patient care and services.
- Advocate for patients and families to ensure their needs are met and their voices are heard.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Bachelor's Degree in Nursing or related field.
- Current California Registered Nurse License.
- 2 years of acute hospital experience, including 1 year of supervisory experience.
- Strong knowledge base or experience in specialty area assigned.
- Master's Degree in Health Care related field.
