Job Summary:

As a Clinical Care Coordinator at Vivo HealthStaff, you will play a vital role in ensuring the seamless delivery of patient care. Your primary responsibility will be to work closely with our Medical Operations team to provide exceptional support to patients. This includes responding to their queries, assisting with scheduling, and utilizing your clinical judgment to promote their health and wellness.

Key Responsibilities:

• Collaborate with pharmacists, physicians, and our Care team to ensure timely and high-quality patient care
• Communicate effectively with patients via phone, video, or secure chats to provide education on treatment plans and answer basic medical questions within your scope of practice
• Coordinate patient scheduling, follow-ups, and blood test ordering processes while serving as a resource for physicians within your scope of practice
• Work with the Medical Operations team to identify opportunities to reduce member and physician pain points
• Collaborate with a team of nurses to provide around-the-clock patient care
• Under the supervision of a physician, coordinate patient care within the medical team
• Assist in training and educating the Care team on new campaigns

Requirements:

• BSN degree in Nursing
• Active, unrestricted RN license in New York and additional states (CA, PA, OH, MA preferred) and the ability to obtain additional state licenses as needed
• 3+ years of experience caring for patients in a hands-on, acute care setting OR 2+ years combined experience in hospitals/outpatient clinics/insurance companies
• Off-the-charts enthusiasm, patience, and empathy when helping our members
• Excellent communication skills
• Savvy with technology
• High attention to detail and a solutions-oriented mindset
• Ability to think critically and quickly, and to work autonomously
• Strong organization skills and the ability to maintain a cool head under pressure
