Position Overview

Assertive Community Treatment (ACT) Service Coordinator - Join a dedicated team focused on transforming lives.

About Pathways Vermont: We are a progressive nonprofit organization devoted to eradicating homelessness and providing cutting-edge mental health support through continuous, compassionate social work.

Role Summary: As a Service Coordinator, you will be an essential part of an interdisciplinary team delivering adaptable, empathetic, and community-oriented services aimed at fostering stability, recovery, and reintegration for individuals facing mental health challenges, histories of homelessness, and potential substance use issues. You will play a crucial role in Pathways' Assertive Community Treatment (ACT) team, collaborating with various professionals to manage a diverse client caseload and liaising with community partners.

Position Availability: 1

Work Schedule: Full Time, 35-40 hours per week

Key Responsibilities:

  • Build and nurture relationships with clients, providing services from a client-centered, harm reduction, and strengths-based perspective.
  • Offer individualized support and guidance on essential life skills, housing issues, and health maintenance, including medication management, housekeeping, and financial planning.
  • Assist clients in navigating the housing process, including unit viewings, move-in support, and landlord negotiations.
  • Coordinate and oversee referrals to community resources, advocating for client engagement and managing communications with external agencies.
  • Develop a comprehensive understanding of community resources and demonstrate a proactive approach to exploring new options as needed.
  • Participate in annual staff retreats and programmatic gatherings, which are integral to employment at Pathways Vermont, unless otherwise excused.
  • Perform additional duties as assigned.

Qualifications:

  • Experience working within a Housing First, client-centered, harm reduction framework.
  • Demonstrated patience, creativity, flexibility, compassion, and sensitivity towards individuals with disabilities and marginalized groups.
  • Strong understanding of the challenges faced by individuals with mental health or substance use issues, particularly regarding basic needs and advocacy.
  • Excellent problem-solving abilities and a knack for intuitively addressing complex situations.
  • A valid driver's license, reliable vehicle for work-related tasks, and current automobile insurance are required.
  • Willingness to engage in self-reflection, learning, and developing new relational approaches.
  • Ability to tolerate risk and discomfort.

Compensation Package:

  • Starting salary of $20-$21, with potential for an 8.5% increase after one year of employment.
  • A $3000 Hiring Bonus is available.
