Job Title: ACT Team Service Coordinator

Pathways Vermont is seeking a highly motivated and compassionate Service Coordinator to join our Assertive Community Treatment (ACT) team. As a key member of our interdisciplinary team, you will provide flexible and community-based services to individuals experiencing mental health challenges, homelessness, and substance use issues.

Job Summary:

The Service Coordinator will engage in relationship building and provide services from a client-centered, harm reduction, and strengths-based approach. You will provide one-to-one support and counseling on life skills, housing concerns, and health maintenance, including tasks such as medication support, housekeeping, and assisting clients with managing their finances.

Responsibilities:
  • Build and maintain relationships with clients, providing services from a client-centered, harm reduction, and strengths-based approach.
  • Provide one-to-one support and counseling on life skills, housing concerns, and health maintenance.
  • Assist clients in navigating the housing path, including viewing units, assistance with moving in, life skill development, and landlord/client disputes.
  • Coordinate and monitor referrals to community services, and advocate for client participation in them.
  • Develop a working knowledge of relevant community resources and demonstrate a willingness to explore new or unfamiliar resources as necessary.
  • Attend the annual All Staff and Programmatic Retreats, unless excused by the employee's manager/HR.
Qualifications:
  • Ability to work within a Housing First, client-centered, harm reduction framework.
  • Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other marginalized populations.
  • A good understanding of the issues facing persons with mental health challenges or substance use issues, particularly as related to meeting basic needs of food, clothing, shelter, and advocacy.
  • Excellent independent problem-solving skills and demonstrated ability to intuitively approach challenging situations.
  • Valid driver's license, a reliable personal vehicle available for work purposes, and current automobile insurance required.
  • Willingness to self-reflect, learn, and practice new ways of being in relationships.
  • Risk and discomfort tolerance.
Compensation:
  • $20-$21 starting; with up to 8.5% increase available after 1 year employment.
  • $3000 Hiring Bonus Available.
