Care Coordinator for Behavioral Health

As a key member of the Health Care and Rehabilitation Services (HCRS) team, the Care Coordinator for Behavioral Health plays a vital role in providing care coordination, case management, and resource connection to individuals and families in need.

Key Responsibilities:
  • Coordinate internal and external referrals to ensure seamless care transitions.
  • Utilize the SBIRT model to identify and address mental health and substance use concerns.
  • Assist individuals and families in identifying and prioritizing goals, and develop time-limited care coordination plans to achieve them.
  • Provide care coordination and resource connection to support individuals in achieving their goals.
  • Establish and maintain relationships with community partners, other providers, and support systems to ensure comprehensive care.
  • Complete periodic reports and maintain accurate and timely clinical documentation in the Electronic Medical Record.
Scope of Position:

The Care Coordinator for Behavioral Health is an integral part of the HCRS team, working closely with other providers and community partners to provide comprehensive care coordination, case management, and resource connection. This role requires a strong understanding of mental health, substance use, and behavioral issues, as well as excellent communication and interpersonal skills.

Requirements:
  • Bachelor's degree in Social Work or related field (or equivalent experience).
  • Ability to engage, empathize, and have compassion for individuals and families in need.
  • Knowledge of developmental trauma and attachment issues, mental health, substance use, and behavioral issues.
  • Healthy communication skills and ability to work well with a team.
