Care Coordinator - Behavioral Health

As a key member of the Health Care and Rehabilitation Services (HCRS) team, the Care Coordinator - Behavioral Health plays a vital role in providing care coordination, case management, and resource connection services to individuals and families.

Key Responsibilities:
  • Coordinates internal and external referrals to ensure seamless care transitions.
  • Utilizes the SBIRT model to identify and address mental health and substance use concerns.
  • Assists individuals and families in identifying and prioritizing goals, developing time-limited care coordination plans, and measuring success.
  • Provides care coordination and resource connection services, establishing relationships with community teams and providers.
  • Maintains accurate and timely clinical documentation in the Electronic Medical Record.
Requirements:
  • Bachelor's degree in Social Work or related field (or equivalent experience).
  • Excellent communication and interpersonal skills.
  • Ability to work well in a team environment and demonstrate flexibility.
  • Intermediate to advanced computer skills.
Working Conditions:

The Care Coordinator - Behavioral Health works in a fast-paced office and community-based environment, with occasional travel to client homes or community locations. The noise level is generally quiet.
