Job Title: Physical Therapist Per Visit Home Care Specialist

About the Role:

We are seeking a skilled Physical Therapist to join our team as a Home Care Specialist. As a Physical Therapist, you will be responsible for providing professional rehabilitation therapy to patients in their homes through evaluation, development, and implementation of individual goal-oriented therapy programs.

Responsibilities:

  • Complete patient evaluations within the specified time frame and ensure they contain data reflective of a complete, holistic assessment of the client's condition.
  • Develop individualized treatment programs based on evaluation findings, focusing on time-oriented, functional, and measurable short and long-term goals.
  • Prepare appropriate system documentation, such as progress notes, reassessments, and discharge summaries, according to policy and procedures.
  • Act as a liaison among physicians, MCPs, and other members of the healthcare team to ensure effective coordination, appropriate resource referral, and utilization of home health aide services.
  • Provide comprehensive education to patients and/or caregivers based on their identified needs in a manner they can understand.
  • Develop a rapport and mutual respect with patients to foster an optimum environment for goal achievement.
  • Implement treatment plans utilizing correct and appropriate clinical skills and techniques.
  • Participate in discharge planning throughout the patient's course of care.

Requirements:

  • Bachelor's Degree in Physical Therapy (PT)
  • Minimum of Six (6) Months PT Experience
  • PC Experience/Skills

Preferred Qualifications:

  • Doctorate

Licenses and Certifications Required:

  • Physical Therapist License
  • AHA Basic Health Care Life Support HCP Certification
