Job Summary

We are seeking a skilled Physical Therapist to join our Home Health team at Hackensack Meridian Health. As a Physical Therapist, you will provide professional rehabilitation therapy to patients in their homes, evaluating, developing, and implementing individualized goal-oriented therapy programs in accordance with organization policies and procedures.

Responsibilities
  • Conduct thorough patient evaluations within specified timeframes, ensuring comprehensive assessments of client conditions.
  • Develop and implement individualized treatment programs based on evaluation findings, focusing on time-oriented, functional, and measurable short and long-term goals.
  • Prepare and maintain accurate system documentation, including progress notes, reassessments, and discharge summaries, adhering to policy and procedures.
  • Ensure timely and precise documentation, supporting the need for continued skilled care in the home.
  • Coordinate with physicians, MCPs, and other healthcare team members to ensure effective care delivery, resource referral, and utilization of home health aide services.
  • Supervise and educate Physician Therapy Assistants, assessing their ability to perform interventions and ensuring co-signatures within regulatory timeframes.
  • Provide comprehensive education to patients and caregivers, addressing their identified needs and fostering an optimum environment for goal achievement.
  • Implement treatment plans utilizing correct and appropriate clinical skills and techniques, meeting productivity requirements consistently.
  • Participate in discharge planning, addressing problem areas and ensuring a smooth transition of care.
  • Complete scheduling requirements accurately and timely, updating and submitting new and changed schedules through appropriate system applications.
Qualifications
  • Bachelor's Degree in Physical Therapy (PT)
  • Minimum of Six (6) Months PT Experience
  • PC Experience/Skills
