BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Seabrook, NH. As a Physical Therapist, you will be responsible for providing high-quality care to our clients in the comfort of their own homes.
Responsibilities:- Conduct thorough assessments and develop individualized care plans to meet the unique needs of each client.
- Provide manual therapeutic exercises, gait training, balance, and other interventions to promote client independence and mobility.
- Collaborate with nursing and other healthcare professionals to ensure comprehensive care and support.
- Educate clients and their families on rehabilitative care and activities necessary for optimal health and safety.
- Accurately document client care management and services provided.
- A current New Hampshire Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Solid computer skills and prior experience with electronic medical records (EMR) preferred.
- Paid holidays, vacation, and sick leave.
