As a Home Health Physical Therapist, you will work independently to assess, plan, and implement individualized care plans to help our clients achieve their rehabilitation goals.

Responsibilities:
  • Conduct thorough assessments of clients' physical abilities and develop personalized care plans to address their needs.
  • Provide one-on-one physical therapy sessions to clients in their homes, focusing on exercises, stretches, and other interventions to improve mobility, strength, and function.
  • Collaborate with other healthcare professionals, including nurses and occupational therapists, to ensure comprehensive care and coordination.
  • Document client progress, including treatment plans, progress notes, and discharge summaries.
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care and address any concerns or questions.
Requirements:
  • Current NJ Physical Therapist license.
  • Minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
