Advanced Practice Provider

We are seeking an Advanced Practice Provider to join our established Hospitalist Service at Yale New Haven Hospital.
This position will provide care to patients at home in our Home Hospital program, working collaboratively with our team to deliver high-quality patient care.
Key Responsibilities:
- Provide acute and restorative care services to patients in their homes
- Collaborate with our team to develop and implement individualized care plans
- Conduct home visits to assess patient needs and provide care
- Work closely with our hospitalist team to ensure seamless transitions of care
Requirements:
- Graduate from an accredited Advance Practice Nurse Practitioner Program or accredited Physician Assistant Program
- 4 years of clinical working experience as an APP within inpatient settings or combination of inpatient and outpatient settings
- Connecticut State Licensed PA or APRN Practitioner License
- Board certified
- ACLS/BLS Certified
- DEA License
