Job Title: Advanced Practice Provider

Yale New Haven Health is seeking an Advanced Practice Provider to join our established Hospitalist Service at Yale New Haven Hospital.

About the Role

This position will provide care to patients at home in our Home Hospital program. Shifts are 8 hours duration Monday to Friday, including one weekend every 4 weeks.

Qualifications

To be considered for this role, you must have:

  • Graduate from an accredited Advance Practice Nurse Practitioner Program or accredited Physician Assistant Program
  • Total of 4 years of clinical working experience as an APP within the inpatient settings or combination of inpatient and outpatient settings
  • Connecticut State Licensed PA or APRN Practitioner License
  • Board certified
  • ACLS/BLS Certified
  • DEA License
