Advanced Practice Provider

Yale New Haven Health is seeking an Advanced Practice Provider to join our established Hospitalist Service at Yale New Haven Hospital. This position will provide care to patients at home in our Home Hospital program.
Key Responsibilities:
Provide acute and restorative care services to patients in their homes
Collaborate with the Hospitalist Service team to ensure seamless care transitions
Develop and implement individualized care plans to meet patient needs
Requirements:
Graduate from an accredited Advance Practice Nurse Practitioner Program or accredited Physician Assistant Program
4 years of clinical working experience as an APP within the inpatient settings or combination of inpatient and outpatient settings
Connecticut State Licensed PA or APRN Practitioner License
Board certified
ACLS/BLS Certified
DEA License
