Advanced Practice Provider Opportunity at Yale New Haven Health

Yale New Haven Health, the largest and most comprehensive healthcare system in Connecticut, is seeking an Advanced Practice Provider to join its established Hospitalist Service at Yale New Haven Hospital.

About the Role

This position will provide care to patients at home in our Home Hospital program. Shifts are 8 hours duration, Monday to Friday, including one weekend (Saturday and Sunday) every 4 weeks.

Requirements
  • Graduate from an accredited Advance Practice Nurse Practitioner Program or accredited Physician Assistant Program
  • Total of 4 years of clinical working experience as an APP within the inpatient settings or combination of inpatient and outpatient settings
  • Connecticut State Licensed PA or APRN Practitioner License
  • Board certified
  • ACLS/BLS Certified
  • DEA License

This position requires travel to patient's homes to provide acute and restorative care services within the home environment.
