Key Responsibilities- Assist with collecting information for individual intake and admission based on interviews with clients, their families, and community agencies.
- Collaborate with the treatment team to identify individual strengths and weaknesses, and develop a treatment plan.
- Monitor progress toward treatment goals and report unusual behavior or physical complaints to direct supervisors.
- Provide direct care and support to individuals, including assisting with daily living activities and leading milieu activities.
- Document observations and vital signs in Carelogic, and participate in educational programs and in-service meetings to enhance professional growth and development.
