Job Summary

The Qualified Mental Health Professional - Community Services (QMHP-CS) is a critical role in providing Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families.

Key Responsibilities
  • Deliver case management and skills-based interventions to increase the youth's natural supports and strengths, fostering resilience and recovery.
  • Provide services in the participant's home or appropriate community setting, ensuring compliance with Texas Medicaid Provider Procedures Manual, Texas Resilience and Recovery Utilization Management Guidelines, YAP standards, and all state and federal regulations.
  • Accurately document services, attend Wraparound Team meetings as needed, and communicate participant progress to supervisors, team members, and the referring authority.
  • Collaborate with families to develop and implement individualized service plans, promoting positive outcomes and empowering youth to achieve their goals.
Requirements
  • Bachelor's or Master's degree in a human service related field, as defined by Texas Administrative Code.
  • Experience in the provision of home and community-based services, with a focus on behavioral health and case management.
  • CPR/First Aid Certification and Behavior Management (MANDT) Certification.
  • Reliable transportation, valid driver's license, and current auto insurance coverage.
