YOUR ROLE IN OUR MISSION:

A Pharmaceutical Care Specialist plays a vital role at AHF by being assigned to multiple locations based on the operational needs of the practice sites within the region of their primary residence. This role expands the primary care responsibilities of pharmacists, including personalized health promotion and disease prevention initiatives, such as vaccine administration, long-acting HIV therapies, diabetes management, smoking cessation programs, and other relevant areas, in collaboration with AHF healthcare providers.

As a Pharmaceutical Care Specialist, you will enhance AHF's reputation by improving patient outcomes, enriching the services offered by AHF, creating opportunities for professional advancement and satisfaction, and contributing to the ongoing development of the AHF Healthcare System.

You will serve as a passionate advocate for our key initiatives.

AHF promotes a collaborative organizational structure where staff members are accountable to multiple leaders. The Pharmaceutical Care Specialist will work within a close-knit team to deliver tailored health promotion and disease prevention services, including immunization administration, diabetes management, smoking cessation programs, and other Subject Matter Expertise (SME) initiatives developed by AHF, with appropriate collaborative drug therapy management authority. This role may include prescriptive authority where legally and organizationally permitted, such as for PrEP and PEP therapies.

Licensure/Certifications: A valid State Pharmacist License in the state of application is required.
