Position Overview

The Patient Care Team Leader plays a crucial role in guiding the Patient Care Team. This individual is responsible for:

  • Supervising, assessing, and coordinating the diverse members of the interdisciplinary team.
  • Ensuring seamless continuity of care from the point of admission through to discharge or transfer, including bereavement support.
  • Acting as an advocate for patients while coordinating with other healthcare and social service providers in the community involved in patient care.
  • Maintaining comprehensive patient records from intake through to discharge or transfer.
Key Responsibilities

The Team Leader oversees all patient care services provided by the team, which includes volunteer support and bereavement services, ensuring these are delivered as integrated components of the interdisciplinary care plan.

Additionally, the Team Leader:

  • Reviews all pending patient admissions, new admissions (including assessments and histories), and discharges to confirm that eligibility and appropriateness criteria are met.

Qualifications

Applicants should possess:

  • At least 1 year of management experience.
  • A minimum of 3 years of experience as a Registered Nurse (RN).
