POSITION TITLE:
HIV/AIDS MEDICAL CASE COORDINATOR

REPORTS TO:
Clinical Supervisors
This role involves providing comprehensive medical case management services for individuals and families affected by HIV/AIDS.

Medical Case Coordinators are expected to uphold all standards of social work practice, educate clients about HIV, facilitate access to necessary medical care and medications, and offer psychosocial support along with referrals to a wide array of services.

A minimum of a Bachelor's Degree in Social Work, an active LSW in Ohio or the ability to secure an LSW license within 90 days, and two years of experience in social work is preferred.

Candidates must demonstrate exceptional skills in interviewing, assessment, service planning, service delivery, and program evaluation. Familiarity with HIV/AIDS service networks, entitlement programs, and community services is essential. Responsibilities include providing medical case management services such as interviewing, assessment, service planning, referrals, follow-up, and evaluation.
Educate clients on safer sex practices, medication adherence, and linkage to medical care.
Participate in individual and group supervision meetings.
Maintain client and statistical records as mandated by social work practice, regulatory or funding agencies, and Caracole.
Represent Caracole within the community.

A minimum of a Bachelor's Degree in Social Work, an active LSW in Ohio or the ability to obtain LSW license within 90 days, and two years' experience as a social worker preferred.

Candidates must possess strong skills in interviewing, assessment, service planning, service delivery, and program evaluation. Basic computer skills, including proficiency with Microsoft Office, are required.
Candidates must be able to read, write, speak, and understand English with excellent verbal and written communication skills.
Sensitivity towards individuals living with chronic diseases (HIV/AIDS and/or addiction) and co-existing social, mental, and physical conditions is crucial.
Bilingual (Spanish/English) is preferred.

This position involves working in the office, remotely from home, and in the community. Responsibilities include using hands and fingers to handle or operate objects, as well as operating a telephone, fax machine, and computer. The employee is frequently required to move throughout the building and to various community sites. COVID-19 vaccination (or waiver) is required upon hire.

This is a full-time position with an hourly wage of $47,000 - $48,000 annually, based on educational qualifications. Benefits include Medical, Dental, Vision, Life and Disability Insurance, and a 401K plan with company matching.
