Role of the Case Manager

The Case Manager at NewPath Child & Family Solutions plays a crucial role in guiding clients and their families toward achieving their defined goals and enhancing their independence and daily functioning through targeted interventions and advocacy. This position involves delivering ethical and therapeutically sound evidence-based interventions, utilizing both internal and external resources to positively influence each client's journey while adhering to established agency policies and procedures.

Key Responsibilities Include:

  • Assessing the needs of each youth and coordinating necessary evaluations and assessments.
  • Facilitating individual, family, and group treatment interventions.
  • Supporting clients in articulating their needs and accommodations across various life domains.
  • Assisting clients in overcoming barriers to achieving their therapeutic objectives.
  • Coaching clients in social and interpersonal skills through role modeling.
  • Monitoring clients' symptoms, functioning, strengths, challenges, and needs.
  • Providing educational resources to help clients understand their mental health diagnoses and strategies for management.
  • Collaborating with involved parties, including family members and other agencies, to connect clients with appropriate resources.
  • Ensuring that all required elements of the electronic case record are accurate, current, and compliant with agency standards.

Qualifications and Skills

  • A High School diploma with a minimum of 5 years of experience working with children with serious emotional disturbances (SED) is required.
  • A Bachelor's degree in a relevant field with at least 6 months of experience working with SED children is preferred.
  • A strong commitment to providing community-based services to a diverse clientele.
  • Knowledge of behavioral interventions and their application.
  • Flexibility to work varied hours, including evenings and weekends.
  • Must be at least 21 years of age.
  • A valid driver's license and proof of current auto insurance with a clean driving record are required.
  • A clear criminal background check is mandatory.
