Job Summary

This position provides medical case management services for individuals and families living with HIV/AIDS. As a Medical Case Manager, you will be responsible for providing comprehensive care and support to clients, ensuring they receive the necessary medical treatment and services to manage their condition.

Key Responsibilities
  • Client Assessment and Service Planning: Conduct thorough assessments of clients' medical and social needs, develop and implement individualized service plans to address these needs, and provide ongoing support and monitoring to ensure clients receive the necessary services.
  • Medical Case Management: Facilitate access to medical care and medications, provide education on safer sex practices, medication adherence, and linkage to medical care, and coordinate with healthcare providers to ensure clients receive timely and effective treatment.
  • Collaboration and Communication: Work closely with Caracole team members, including clinical supervisors, to ensure seamless coordination of services and effective communication with clients and their families.
  • Documentation and Reporting: Maintain accurate and up-to-date records of client interactions, service plans, and outcomes, and provide regular reports to Caracole management on client progress and program effectiveness.
  • Professional Development: Participate in ongoing education and training to enhance knowledge and skills in medical case management, HIV/AIDS care, and social work practice.
Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work (MSW) or a related field.
  • Experience in medical case management, HIV/AIDS care, or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and customer service.
  • Proficiency in Microsoft Office and electronic health record systems.
